Our Payment Policies

co-payments, co-insurance and deductibles

All co-payments, co-insurance, and deductibles must be paid at the time of service. This is part of your contract with your insurance company. If a required amount is not collected at the time of service, we may charge the credit card on file within 24 hours of your visit.

PROOF OF INSURANCE

All clients must complete our registration form before seeing a provider. We require a copy of your current valid insurance card. If correct insurance information is not provided in a timely manner, you may be responsible for the full cost of services received.

COVERAGE CHANGES

If your insurance changes, please notify us before your next visit so we can update your information and help you receive your maximum benefits. If your insurer does not make timely payment, we will encourage you to follow up with them directly.
